12. Police Clearance Certificate

Requirements for Fresh Passport

1. Tatkal SCHEME: The applicant's passport size photograph is also required to be affixed on the Verification Certificate(text available in this site) and attested by the officer issuing the Verification Certificate with his/her signature and rubber stamp in a Letter head in such a way that half the signature and stamp appear on the photograph and half of the certificate.
   
   
2. For proof of stay applicant should produce any ONE of the following documents  
   a. Family Card(popularly known as ration card or sugar card) (OR)
   b. Bank Pass Book with minimum operation of One Year   (OR)
   c. Telephone Bill for the last two years (OR)
   d. Gas Connection Book with full entry for two years (OR)
   e. Identity card issued by the Chief Election Commissioner
   


3. For Proof of Date of Birth applicant should produce any ONE of the following documents
   a. Transfer Certificate from Educational Institution (OR)
   b. Birth Certificate issued by Municipality/Corporation (OR)
   c. Bonafide Certificate from the school (OR)
   d. Tenth Standard Mark Statement (OR)
   e. Twelfth Standard Mark Statement

1. Election Commission of India Identity Card
2. Family Card (popularly known as Ration Card)
3. Bank Pass Book

1. Service Identity Card issued by State/Central Government/Public Sector Undertakings/Local Bodies/Public Limited Companies
2. SC/ST/OBC Community Certificates
3. Freedom Fighter Identity Cards
4. Arms Licenses
5. Property documents, such as Pattas, Registered Sale Deeds
6. Pension Documents such as Ex-Servicemen’s Pension Book/Pension Payment Order/Ex-servicemen’s Widow/Dependent Certificates/Old Age Pension Order/Widow Pension Order
7. Railway Identity Cards
8. Income Tax Identity (PAN) Cards
9. Student Identity Cards issued by Recognised Educational Instiutions
10. Driver’s Licenses
11. Birth Certificates issued under the Registrar of Births & Deaths Act

1. Birth Certificate or Bonafide Certificate in School Letter Head
2. Parent's Original Passport
3. If either father or mother doesn't have passport then they have to execute an Affidavit for Minor on a Non-Judicial Stamp Paper Affidavit by Legal Guardian (in case the parents are not legal guardians) (text available in this site)
4. In this case, apart from an affidavit by the legal guardian two responsible persons known to the legal guardian should also execute an affidavit.
5. In case where the parents are resident outside India, such consent from the parent( s), in the form of Sworn Affidavit, dully attested by the Indian Mission abroad, is acceptable. At least one parent of the minor child must hold a valid Indian passport, failing which passport to minor applicant shall be issued only after police clearance.
